Instructions for Firearms Applicants

 

Follow the instructions below and return all completed paperwork to the Matawan Police Department Records Division located at 150 Main Street

Monday through Friday 8:00 a.m. – 4:00 p.m.   

Applications will only be accepted during these hours.

The Records Division is closed from 1:00 p.m. – 2 p.m.

Application for Initial Firearms Purchaser I.D. Card

  1. Complete the Firearms Purchaser I.D. Card Application (STS-33) found at: http://njsp.org/info/pdf/firearms/sts-033.pdf.  Check the appropriate boxes at top of application. Complete the application in its entirety.  If hand written, please print legibly. Answer all questions fully and provide complete mailing addresses with zip codes for those individuals listed as references. Do not use a relative as a reference.

Ø  Application for a permit to purchase a handgun may be made, using the same application, at the time you complete your initial firearms purchaser identification card. 

  1. Complete a Consent for Mental Health Records Search Form (SP-66) found at: http://njsp.org/info/pdf/firearms/sp-066.pdf
  2. Once the application is submitted to the Records Division, you will be given a Matawan Police Department Case number, and an Identogo by Morphotrust USA form.  You will then be instructed to make an appointment to be fingerprinted.  The cost for the fingerprinting is $57.20 paid online.
  3. Once processing of your application is complete, there will be a $5.00 fee for an initial Firearms Purchaser I.D. Card and a $2.00 fee for each permit to purchase a handgun (if applied for). The fee(s) can be paid in cash (exact change) or by personal check.

 




Application for permit to purchase handgun, or replace a damaged, lost or stolen identification card
(you must be 21 years of age to purchase a handgun)

 

  1. Complete the Firearms Purchaser I.D. Card Application (STS-33) found at: http://njsp.org/info/pdf/firearms/sts-033.pdf.      Check the appropriate boxes at top of application. Complete the application in its entirety.  If hand written, please print legibly. Answer all questions fully and provide complete mailing addresses with zip codes for those individuals listed as references. Do not use a relative as a reference.

Ø  Application for a permit to purchase a handgun may be made, using the same application, at the time you complete your initial firearms purchaser identification card.

Ø  Applicants who currently possess a Firearms Purchaser I.D. Card must also complete a criminal history record information request for a non-criminal justice purpose form if they wish to:

 

ü  Replace a current lost or stolen identification card.

ü  Replace a mutilated identification card.

ü  Change an address on a current identification card.

ü  Change their name on identification card.

ü  Change of Sex on identification card.

ü  Apply to purchase a handgun(s).

This is completed by:

1.      Logging onto: https://www.njportal.com/njsp/criminalrecords/

2.     Click on the ON LINE FORM 212A, a highlighted block located on the lower left side of the page.  Enter the nine digit Originating Agency Identification Number (ORI) NJ0132900

3.     Follow the prompts for demographic and payment information. There is a $20.00 fee for the submission. Upon completion of the form the applicant will receive an email confirmation & receipt that will include a confirmation number. Please bring a copy of the email confirmation receipt when dropping off your application. 

§  The request will be forwarded to the Police Department’s work queue for approval and submission to the NJ State Police for processing.

§  Once processing of the application is complete there will be a $5.00 fee for an initial Firearms Purchaser I.D. Card and a $2.00 fee for each permit to purchase a handgun (if applied for). The fee(s) can be paid in cash (exact change) or by personal check.

 

 

ü  Once your application is processed and we have received your submitted information from the state and references, you will be notified by telephone once your application has been approved and your paperwork is ready for pick up. 

In the case of a potential denial, you will be scheduled to interview with the Chief of Police.
